In order to (continue to) ensure the quality of our services for you and to provide our services as efficiently as possible, we ask you to observe the following "house rules" below.
At the first appointment, we ask that you bring a valid form of identification showing your BSN* (Citizen Service Number). This can be a valid passport, European identity card or driver's license. Are you an asylum seeker? Then bring your RZA pass, passport and alien's document. If you have moved or are changing address, please let us know as soon as possible. This also applies to a change of health insurance company, telephone number or e-mail address.
We are required by law to perform a BSN registration and identity check. This is stated in the Law on the use of Citizen Service Number in healthcare. More information can be found on the site of Authority Personal Data.
For your safety and in the interests of treatment, it is very important that you inform us at each visit of any medical details that you have not (yet) communicated. Pregnancy, medication use, illnesses, allergies, physical and mental deviations among others may affect your health. Because of this, it is possible that it may involve a risk.
At each treatment, the acting specialist will ask you about medical changes and details. This also happens when you have two treatments in one day. This may seem strange. We do this not only out of obligation to ask, but also to ensure the quality of the treatment as much as possible.
Patient information is not sent or told to third parties without consultation with the dentist and with the patient's knowledge. All information remains within The Delft Dental Practice, except if it is information for a specialist or family doctor, who needs the information for medical research (referral). If your patient information is requested by another doctor, health care practice or family members we always ask you to give written permission for this. This may also be done digitally, but by using the e-mail address known to us.
Are you moving and choosing a new dentist? Then we ask you to complete a release form. With this form, you grant us permission to release and forward your patient information. You can request this form at the front desk.
Making an appointment is easy. You can call us within our business hours or via our website www.delftsetandartspraktijk.nl fill out the contact form. We will then call you back within 24 hours. In addition, you will find more information about our treatments and practitioners on our website.
You can visit us Mon. to Fri. from 9:00 am to 5:00 pm and Sat. from 10:00 am to 5:00 pm.
When making an appointment, you will immediately receive an e-mail with the appointment confirmation. Should you not have received this e-mail, we would like to ask you to check your Spambox in the first instance. Should you still not find it, please feel free to contact one of our colleagues. Do you prefer to have a physical appointment card? Ask at the front desk so you can get one to take with you.
Two days in advance you will receive an email/text reminder for your appointment. This is an additional service and does not replace the regular appointment procedure. Do you not receive a text message or e-mail for whatever reason? Then you cannot derive any rights from this and we will still expect you on the pre-arranged date and time.
Cancel or reschedule an appointment we receive at least two business days in advance. We operate a 48-hour schedule based on 2 business days. This is to try to fill the schedules of our practitioners as efficiently as possible. You must always inform us of the cancellation by telephone. This way we can immediately discuss your situation and make a new appointment with you if necessary. Outside working hours, you can email or app us at our phone number +31 (0)15 202 97 33.
Do we receive your cancellation late or do you not arrive at the appointed time without a valid reason? This can damage the relationship of trust. We may charge for the reserved time. We thank you for your understanding.
