Definitions

Practice means The Delft Dental Practice.
Care provider means the attending dentist or orthodontist in the practice.
Patient means the natural person receiving care.
Agreement means the arrangement for healthcare delivery between healthcare provider and patient.

Applicability

These terms and conditions apply to all care and payments between practice and patient.
By signing the treatment agreement, the patient accepts these terms and conditions.

Treatment Agreement

The provider and patient enter into a treatment agreement for dental care.
The practice provides care according to professional standards.
Patient provides accurate information about health and any payments.

Payment

Invoices are sent through Payt.
The patient pays within the payment period on the invoice.
The payment period is 14 days from the invoice date.
Payment is made through the payment options on the Payt invoice.
In case of late payment, Payt sends reminders.
Persistent arrears may lead to collection measures.

Costs and fees

Rates follow the practice's current fee schedule. Any additional charges for prosthetics, materials or laboratory work will be billed separately.

No insurance

Health insurance is not part of the patient-practice agreement.
Patients must arrange their own health insurance and reimbursement.

Cancellation and no show

Appointments must be cancelled at least 24 hours in advance.
For failure to cancel on time, the practice may charge a fee.

Applicable law

These terms and conditions are governed by Dutch law.
Disputes will be submitted to the competent Dutch court.
